Billing & Payment Information

We accept most insurances, including Medicare, Medicaid, Medical Assistance, Blue Cross Blue Shield, Medica, Blue Plus, IMCare, United Behavioral Health, Optum, Health Partners, HealthEZ, TriCare, UCare, Cigna, Amerigroup, and Preferred One. If you don't see your carrier listed here, contact the number on the back of your insurance card to inquire if we are in-network.

Creative Solutions Counseling proudly serves all patients regardless of race, color, sex, national origin, disability, religion, age, sexual orientation, gender orientation, or inability to pay. As an approved NHSC-site, we are committed to making mental health care accessible and affordable for all by offering a Sliding Fee Discount Program. Discounts for core comprehensive behavioral health services are offered based on family size and income. For more information, speak to your CSC therapist and complete the application below.

Initial/Annual Assessments
Initial Intake and Diagnosis Evaluation: $ 225/per eval
Extended Diagnostic Evaluation (children 2-5 yrs):$ 300/per Eval.
Extended Diagnostic Evaluation (Adult): $ 350/per Eval.

Individual, couples, or family counseling
30-min session: $ 115
45-min session: $175
60-min session: $200
90-min session: $400
Specialty/Add-on (EMDR, Alpha Stim, etc.): $25

 Additional Services NOT Covered by Insurance
Letter/Reports requested by parent or courts: $ 200/per request
Court Appearance: $ 250/hr. minimum of 2 hours: $ 500/per appearance
We realize that medical bills can be complex, and we want to help you understand your bill and insurance coverage. Prices listed in the fee schedule are adjusted based on your particular insurance plan. If the charges will be paid by an insurance company, they will be billed first. If you are covered by more than one policy, the insurance companies will determine how to divide payment. You will not receive a statement until the insurance company makes payment. Any remaining balance will be your responsibility. 
CSC currently manages all billing services internally. If you have questions about outstanding balances or making a payment, please contact your clinician directly. For any questions regarding your insurance coverage—such as contracts, deductibles, or copays—you will need to reach out to your insurance provider.

No Surprises Act

Under the No Surprises Act, healthcare providers must provide a Good Faith Estimate of expected charges before patients receive care through the Good Faith Estimate (GFE) for uninsured and self-pay patients (including those who choose not to use their insurance).
The No Surprises Act (NSA) is a U.S. federal law enacted in December 2020 as part of the Consolidated Appropriations Act of 2021 and effective January 1, 2022. It shields patients from unexpected out-of-network medical bills—often called “surprise bills”—and sets national standards for price transparency in health care billing.
The act matters because it curbs a widespread cause of medical debt, ensuring patients pay only in-network cost-sharing amounts for covered emergency and certain non-emergency care.

Patient Rights
You have the right to receive a Good Faith Estimate for the total expected cost of care.
If your final billed charges are at least $400 more than the Good Faith Estimate, you may initiate a dispute resolution process through the U.S. Department of Health and Human Services.
​You may request an updated estimate at any time prior to receiving services.
